• Your IP address - -snip-
• The date you were banned - 1/2/2017
• The staff member who banned you (If it was the system write 'System' and if you don't know write 'Unknown') -  system
• Extra space for you to explain what happened - there was a bug with 2 Presidents,and also the 2 Vice Presidents. After they killed me,i got ban. ?

Actually reading the 'how to' makes a difference, who knew!
